This form is required for any student who desires to play sports. CBCA offers Soccer, Basketball & Volleyball. This form must be filled out by a physician and handed into the school office, BEFORE AN ATHLETE CAN PRACTICE!
This form is also required for those students entering Pre-K; K5, 1st, 3rd, 5th, 7th, 9th, and 11th grades

If your student will need to ride a public school bus to schoool, please fill out this form and RETURN IT TO THE SCHOOL OFFICE.

This form is needed for all students. It is needed in case a student is off campus during a school function, and needs medical attention.

This form is used so that the school nurse may dispense medicine to your child during the school day. It must be signed by a Licensed Health Care Provider.
