Below are links for policies and downloadable forms for use at CBCA.

  Please download and print out the Parent-Student Handbook. This version has been approved for the 2005-2006 school year.

• Medical Request Form (Word document, opens in a new window)
  Please download and print out the Medical Request Form if you would like to permit your child to receive selected medications during school hours.
  
• Bus Transportation Request (Word document, opens in a new window)
  Please download and print the Bus Transportation Request for the 2007-2008 school year and return to the office complete by March 19, 2007.
  
• Physical (Word document, opens in a new window)
  Please download and print out the Physical form and take it to your pediatrician. Your child's doctor should complete this form. This is required by the Chenango Forks School District, who is charged with overseeing compliance for CBCA in this area. This form is required for any student participating in sports or entering grades Kindergarten, Second, Fourth, Seventh, or Tenth.
